Something tells me there might have been a failure on the part of the engineer/​driver for not seeing this. They’re required to remain at their window and looking back down the track as the train is exiting the platform for this exact reason (AND why you should never stand in the yellow zone at the platform edge so they have a clear view). Either they somehow missed this or were negligent and didn’t look back is up for debate (I believe that DOT is investigating).

Either way, god, what a terrible thing to witness or have happen to anyone.
